Centegra expands usage of network to all discharges and Medicaid population

Centegra Health System has announced that they are expanding the usage of Dina to manage all discharges from the health system. After seeing significant results with their high-risk population, Centegra will start onboarding all of their preferred post-acute partners onto the platform – a growing digital network connecting care providers from hospital to home. The next phase of the partnership will be focused on streamlining discharge management to increase efficiency with post-acute partners and steer patients to the correct care setting for more cost-effective care.

Expanding access to all of our care managers and our preferred post-acute network will help us gain efficiencies, lower costs and increase quality of care for our patients,” said Astrid Larsen, Director Care Coordination at Centegra Health System.

Centegra originally partnered with Dina in late 2016 to manage its Bundled Payments for Care Improvement (BPCI) population. By facilitating enhanced communication between the hospital and their preferred provider network, they were able to lower readmissions significantly and capture a $1 million net swing in CMS revenue.
