Redeemer Health (formerly Holy Redeemer) uses Dina’s technology to connect its Home Care and Hospice services located in Philadelphia and New Jersey. The goal is to improve transitions and increase visibility as patients are discharged from the hospital and head home. Michelle Walsh, interim lead, care management, calls Dina a “game-changer” for the care management team, especially during Covid, when they were able to track the progress of some patients in their homes and communities. Here’s what else she had to say: 

Tell us about the Redeemer Health-Dina partnership.

Our post-acute, home, and community-based care providers are now connected on the platform so everyone can collaborate in real-time around patient care. This is especially helpful when multiple providers are involved post-discharge. We want to make sure our patients continue to receive quality care as they move outside of the hospital. It also helps us reduce readmissions. If a patient is not feeling well, and if it’s not emergent, we have the opportunity to intervene in the community prior to the patient coming back to the hospital. 

Dina has been a game-changer for our care management team. The technology is user-friendly and we can quickly and efficiently refer patients for care to our partners who are connected on the platform.

How has Dina improved your workflow?

Dina has been a game-changer for our care management team. The technology is user-friendly and we can now quickly and efficiently refer patients for care to our partners who are connected on the platform. Previously, we were printing and faxing our referrals which included a lot of back and forth phone calls. Dina automates the process so we can send everything through the platform immediately, which is easier, more efficient, and more secure. And, we still have the opportunity to work with our partners who are not yet on the platform. 

We also can send multiple referrals at one time to our downstream providers including visiting home care, DME providers, ambulance companies, and behavioral health organizations. As the platform grows, we can grow with it and it will be an important communication tool between the hospital, our providers, and patients.

What’s next for Redeemer Health and Dina?

We want to continue to build a strong referral flow from the hospital to our home health agencies and preferred partners so we can ensure patients continue to receive quality care. We activated two of our LifeCare Residential communities on the platform so we can create a consistent dialogue around the patient, whether they move to skilled nursing or to their home post-discharge. The next phase is to connect our two other LifeCare Residential communities for a completely connected experience. And we look forward to being able to send documentation to payers through the platform for authorizations and review. This becomes more important as care continues to move outside the hospital and into the home and community
