Adds new technology to improve care coordination with area Hospitals, Post-Acute Care and Home-based providers across the care continuum

PreparedHealth announced their partnership with Holy Redeemer Home Health – a growing digital network streamlining care for patients from Hospital to Home. Holy Redeemer, which serves southeastern Pennsylvania and 12 counties in New Jersey, will leverage the technology to build a preferred post-acute care network with their home health and Life Care agencies along with key partner providers. With their emphasis on providing a continuum of care, the Dina partnership will enable them to provide faster, more transparent care, while streamlining care transitions amongst their own locations and those of partners.

Prepared Health network is a powerful community that connects everyone involved in a patient’s Post-Acute Care (PAC), enabling real-time, HIPAA-compliant, and sometimes life-saving communications at the speed of touch. With the technology, post-acute care providers are seeing an increase in referral acceptance, faster transition times and reduced readmissions for upstream partners.
