Jefferson Health, a 14-hospital system in Philadelphia and New Jersey with $5 billion in revenue, named Prepared Health its digital technology partner.

Jefferson Health has selected Prepared Health to be its digital technology partner for post-acute and transitional care.

Chicago-based Prepared Health has developed a technology platform that allows hospitals to connect, collaborate and react in real-time with the providers handling a patient’s care after the patient is discharged.

Jefferson plans to launch the digital care coordination platform at its South Jersey hospitals in Cherry Hill, Stratford and Washington Township. Financial terms of the deal are being kept confidential.

“It’s up to healthcare professionals to use technology to get care to where patients are, instead of getting patients to where care is located,” said Dr. Stephen K. Klasko, president of Thomas Jefferson University and CEO of Jefferson Health.

Klasko said enhancing the health system’s connections with its post-acute providers supports Jefferson’s mission of providing “health care with no address, which underpins our model of health care innovation across the nation.”

Prepared Health’s cloud-based platform connects caregivers including: skilled nursing facilities, home health care agencies, non-medical home care agencies, durable medical equipment providers, pharmacists, geriatricians, family caregivers, and companies that address social determinants of health such as housing and access to nutritious food. Its technology is designed to reduce communication gaps, in real time, that can occur when multiple providers are involved with patient care post-discharge.

The intent of the collaboration, the organizations said, is to reduce readmissions, curb uncompensated care due to unnecessary ER utilization, and streamline patient transitions to post-acute and the home.

Ashish V. Shah, CEO of Prepared Health, said the goal of the partnership is to provide high quality patient experiences “as care continues to move outside of the hospital and into the home.”

Prepared Health and Holy Redeemer Health System in February announced they were expanding their existing relationship.
