“We are joining with BAYADA to create data-driven engagement and bring those experiences into health care,” said Shah. “We want to create a more holistic approach in how we support someone’s care.”

The two companies have actually been working together for a number of years on other projects, beginning with a pilot for high-risk heart patients in New Jersey, which resulted in patient readmissions dropping from 40% to 11%. Shah said they will continue the work they’ve started and take it across the entire BAYADA network, which includes more than 300 offices in 22 states.

“BAYADA has been an incredible partner for us,” said Shah. “We know each other well and we are culturally aligned.”

As PreparedHealth takes steps into the public eye, Shah said the company will focus on bringing attention to the importance of home health care.

“We think part of the challenge is trying to activate the person not doing well, but also activating the family and community members to keep them well,” Shah said. “We believe this is the future of health care.”
