At the start of the pandemic, Philadelphia-based Jefferson Health wanted to move more care to the home to preserve hospital beds for predicted surges and acutely ill Covid patients. Prior to the health crisis, Jefferson had started developing a Hospital-in-Home program to position itself for value-based payment changes. At the same time, the 14-hospital system was managing a growing number of people with chronic conditions who needed longitudinal care management to avoid episodic care.

Creating care-at-home solutions

Today, more than 100 hospitals and 40 health systems have been approved for the CMS Acute Hospital Care at Home waiver program that allows Medicare payments for additional services provided in the home. But most hospitals are still trying to determine whether to implement care at home, how to successfully operationalize it, and how to meet all hospital conditions of participation.
