At the same time, the 14-hospital system was managing a growing number of people with chronic conditions who needed longitudinal care management to avoid episodic care.

Today, more than 100 hospitals and 40 health systems have been approved for the CMS Acute Hospital Care at Home waiver program that allows Medicare payments for additional services provided in the home.
