As the coronavirus continues to spread across the U.S., we’re working with our hospital and post-acute partners to ramp up home-based remote monitoring solutions. We know that hospitals are short-staffed and managing more patients due to the pandemic. In the midst of these challenges, here’s how we’re helping our customers keep their patients and care teams safe:

AI Helps Jefferson “Manage By Exception”

Jefferson Health in Philadelphia has remotely extended care to nearly 1,900 Covid-positive patients, and managed more than 24,000 “digital dialogues” to track symptoms and recovery, and give people a sense of connection with their care teams. Using AI technology, patients who require immediate attention are automatically flagged for direct follow up and further assessment.

Jefferson has activated our new PAC Bed Availability solution to automatically identify the number of Covid-positive and Covid-negative beds in their SNF network. Real-time access to this data helps case management teams ensure timely discharges and appropriate placements.

In addition, the hospital has identified new opportunities to extend care to more patients outside of the hospital, including patients with complex conditions such as Congestive Heart Failure and Chronic Obstructive Pulmonary Disease (COPD).

Caregivers Value Engagement

Since the start of the pandemic, BAYADA Home Health Care found that proactive Covid safety planning helped them improve client acquisition and staff retention.

The home healthcare provider shared its seven biggest lessons learned about keeping patients and caregivers safe. Number one? Caregivers value engagement.

Automation Improves Screening Process

We’ve helped Evolution Health screen more than 3,300 employees, automate a tedious paper record-keeping process, and free up its clinicians and operators to spend more time on patient care. “Dina’s screening solution helps us create a safe work environment and a safe patient-care environment,” said Tom Wilken, Evolution’s vice president of human resources.

Since the start of the pandemic, we have proactively managed nearly 2.5 million health screenings for hospitals and home health agencies across the country. We’re ready to help you create a proactive caregiver and patient engagement plan. Please reach out if you’d like to learn more.
